New Patients
Welcome! It's easy to set up a new patient appointment. We use an online scheduler, and electronic health records which means you fill out forms online with no downloading or printing.
If you are a New Patient, use our Online Scheduler and select "Acupuncture - New Patient". Please include your date of birth in the NOTES of the new appointment. You'll need this for filling out your health history.
(If you do not include your birth date, we'll temporarily use the first day of the current year, and you can correct that when you create your patient portal access. Ex. 1/1/2015)
Our office will then send you a link via the email you use to sign up on the scheduler. Follow that link to our Patient Portal where you can fill out the forms online which get automatically and securely updated to your patient record. We think it's pretty cool and hope you do too.
Having your forms all filled out before you arrive saves us a bunch of time for your first appointment. If you arrive without them filled out, it's unlikely we'll have time for any acupuncture treatment during that initial visit, but you'll still receive a consultation.

If you're planning on using your Insurance, we're happy to verify your benefits for you as a complimentary service to new patients. Please allow several business days in advance of your first appointment, or you may required to temporarily pay out of pocket until benefits are verified.
